What Our Mentoring Program Offers

Our mentors provide consistent, one-on-one support that goes beyond a single session. Here's what you can expect:

  • Support with setting personal goals and creating a clear plan to achieve them

What Can Mentoring Help With?

Mentoring at Ozsom Care is flexible and person-centred — your mentor works with you on what matters most to you. Common areas we support include:

How Ozsom Care Supports You

What makes our Mentoring program different is the depth of the relationship. We don't just assign a support worker — we match you with a mentor who genuinely invests in your journey. Our approach includes:

  • Careful Matching — we take the time to pair you with a mentor who suits your personality, goals, and communication style
  • Consistent Support — the same mentor, session after session, so you can build real trust and momentum
  • Community-Based Sessions — mentoring happens out in the real world, not just in an office, so skills are learned in context
  • Strength-Based Approach — we focus on what you can do and build from there, never on limitations
  • Registered NDIS Provider — fully accredited, with every session documented and aligned to the goals in your NDIS plan

NDIS Funding

Mentoring support at Ozsom Care can be funded through your NDIS plan under the Capacity Building: Improved Living Arrangements, Increased Social and Community Participation, or Finding and Keeping a Job categories depending on your individual goals. Our team will work with you and your support coordinator to identify the right funding line and ensure every session delivers real, measurable value toward your plan goals.
